Referring Providers

Referring providers are the point of contact to the community and our organization is available to help your under-insured patients as a an affordable resource.

Referrals

If you would like to refer a patient to us you may :
1) Call our appointment line during business hours at 1-888-996-0650
2) Fax us a referral order in any format at 855-345-5222

Please ensure that any patients being referred list the referring physician so that reports are correctly forwarded.

Laboratory and Draw Locations

Our program is possible today due to the relationships we have with laboratories and draw locations. They are the backbone of our program and we are always looking add new labs and facilities.

If you are a laboratory or medical office that performs lab tests or a draw location and would like to be a part of our program, please contact us via email at netdev@assisthealthgroup.com

If you have received a letter from our organization, please include the Reference ID that can be found at the bottom of the letter with your correspondence.
